Please note: The Community Connectors were previously called Community Care Navigators.
The project has been commissioned by the East Lancashire Clinical Commissioning Group (ELCCG) and will cover Hyndburn and Ribblesdale. The service provided is intended to reduce the number of referrals of patients to GPs who would benefit from:
- Community Activities including Supported Access
- Socio/Economic Information, Advice and Guidance
- Confidence Boosting
- Supported signposting to appropriately qualified services
Each referral will entail a one-to-one assessment of the client by a Community Connectors. This may done in the client’s own home, at the GP’s surgery or any other suitable venue that both the Connector and Client can agree on together. Referrals will not only come from GP’s but through self-referral and via other agencies who may be in contact with the client, this could include:
- Neighbourhood Integration Team & Community Matrons
- The Fire or Police Authority
- Other advisory agencies, charities or organisations
The Connector Team is made up of 3 part time Co-ordinators working a total of 65 hours per week who will also recruit and train suitable volunteers to provide a joined up service for the benefit of the clients. The individual roles for volunteers are varied, and applicants are asked to contact us to discuss them. The key to the success of the Team will be communication with each other and as a whole team, sharing experiences and best practice, making a commitment to the project and helping each other to help the clients.
